HI Bill, No idea where to get those labels. I couldn't locate our Fatdog's site for it. So I made it at work and laminated it. They are tiny...

I copied the image from the net, unfortunaley ....the quality was not good at all to make it bigger. So, I had to settle with a reasonabley small one. Made a color print and laminated it.
